Community Living Group Home
Community Living - Group Home is a 24/7 residential service that provides structured support to adults with intellectual and developmental disabilities in a provider-owned or provider-rented property. Unlike Supported Living (where the individual holds their own lease), this service operates in a licensed, shared neighborhood home—typically hosting 3 to 8 individuals. Dedicated Direct Support Professionals (DSPs) remain on-site around the clock to ensure personal safety, build independent living skills, and maximize community participation.
Core Areas of Support
24/7 Supervision and Assistance
Offering awake or asleep overnight staffing based on the specific safety and medical profiles of the residents.
Activities of Daily Living (ADLs)
Providing hands-on assistance with personal hygiene, bathing, dressing, grooming, and specialized mobility assistance.
Medication Administration
Utilizing certified staff (such as Certified Medication Technicians) under delegated nursing protocols to manage daily prescriptions.
Health and Appointment Coordination
Coordinating medical appointments, managing nutritional/dietary programs, and maintaining healthcare tracking.
Financial Management Assistance
Helping residents build minor personal budgets and manage day-to-day transaction safety.
Community and Meaningful Day Access
Facilitating transit and support for recreational outings, social clubs, or secondary day services like Community Development Services (CDS).
